Lombok was hit by another natural disaster on Thursday, the third in a little over a week, causing panic, injuries and damage to buildings.
"Evacuees and people ran out of houses when they felt the strong shake of the 6.2 magnitude quake", Sutopo Purwo Nugroho, a spokesman for Indonesia's disaster mitigation agency (BNPB), said on Twitter.
Nugroho says the death toll is expected to rise as search and rescue crews reach more affected areas and begin to search the rubble for more victims.
Authorities said all the tourists who wanted to be evacuated from three outlying vacation islands due to power blackouts and damage to hotels had left by boat, some 5,000 people in all.
According to Indonesia's national disaster agency, the death toll from Sunday's quake stood at 131 as of Wednesday, but local media are reporting figures as high as 381.
United States quake monitors said the latest aftershock had a magnitude of 5.9.
A Reuters witness said the latest quake sent people into the streets in panic and caused buildings to collapse.

The official death toll from the initial quake stood at 321 as of Friday, according to Indonesia's mitigation agency (BNPB).
Almost 68,000 homes were damaged or destroyed in Sunday's quake and 270,000 people are homeless or otherwise displaced, according to the disaster agency's latest update. The quake split apart roads near the coast and sparked landslides that cut off routes used by emergency medical workers and rescue crews who are still trying to clear rubble and recover the dead.
Indonesia's chief security minister had earlier said 319 people had died, while local media have reported figures as high as 347.
Many frightened villagers are staying under tents or tarpaulins dotted along roads or in parched rice fields.
Other evacuees said they were subsisting on a diet of instant noodles and needed clean water and bedding. "It forced us back to this (tented) shelter again", he said.
The Indonesian archipelago is situated along the so-called "Pacific Ring of Fire", an area known for its intense seismic and volcanic activity, which produce about 7,000 earthquakes each year, most of which are of moderate magnitude.
