Outcry over Trump's cuts to Medicare hospital payments

President Donald Trump proposed slashing Medicare funding by $845 billion over the next decade in a budget blueprint by increasing audits of Medicare Advantage payments, changing hospital reimbursement rates and reducing "fraud and abuse" in the system.

Once in the White House, Trump reneged on his Medicaid promise, and now he's being criticized for proposing steep Medicare payment cuts in his new budget.

President Trump's latest budget proposal retains a controversial $8.6 billion request for a border wall with Mexico, while calling for swingeing cuts into government-sponsored National Institutes of Health research including cancer.

The head of a major hospital association says in a blog that the impact on care for seniors would be "devastating".

"Hospitals are less and less able to cover the cost of care for Medicare patients; it is no time to gut Medicare", said Chip Kahn of the Federation of American Hospitals.

Defending the plan, he said: "This budget contains almost $2.7 trillion in savings, more spending reductions proposed than any administration in history". Because of a tax cut for the super-wealthy that created a deficit of $1.9 trillion, and now they've got to go make somebody pay for it.

The plan to winnow funding away from Medicare and Medicaid contradicts campaign promises Trump made in 2016.




HHS secretary Alex Azar will appear in front of three separate Congressional committees this week to defend the budget: Tuesday, the House Energy and Commerce subcommittee on Health; Wednesday, the House Appropriations subcommittee; and Thursday the Senate Finance Committee.

The latest 2020 budget proposal from Trump also outlines a substantial funding increase for the FDA - although commentators are already saying that the plan faces nearly certain rejection from Democrats controlling the US House of Representatives.

The Trump administration has said the plan represents an attempt to be fiscally responsible at a time of trillion-dollar budget deficits. It also wants to slow the growth of payments for skilled nursing, inpatient rehab facilities and home health services, said Tricia Neuman, director, Program on Medicare Policy at the Kaiser Family Foundation. That includes shifting roughly $269 billion in funding for training medical residents and payments to hospitals for caring for the uninsured to the Department of Health and Human Services' budget.

Advocates for Medicare beneficiaries had a measured reaction.

At the moment the budget will be debated - probably in mid-October - presidential campaigns will be intensifying and Trump's interests may be closer to the primaries than to actually getting something out of Congress.

Also, as expected, the president intends to cut $220 billion in the Supplemental Nutrition Assistance Program (known as SNAP or, more commonly, as food stamps), $21 billion in Temporary Assistance for Needy Families, $207 billion in the student loan program and seeks up to a 31-percent reduction in the Environmental Protection Agency.
